The Recovery Coach is responsible for developing, coordinating, and implementing services for adult consumers as identified in the goal-oriented service plan. This role includes providing case management, service planning, and coordination of services with other agency providers to meet the consumer needs and increase community stability in consumer's life. The Recovery Coach will: This position is Community-based and requires travel to homes, and other community locations to provide services and support clients. Mileage reimbursement provided.
