The post holder will work collaboratively across the integrated care system to develop a Social Work Practitioner response within Adult Social Care in order to: This is a public/customer-facing role, where the statutory English language requirement for public sector workers applies. This position will be subject to an enhanced criminal record disclosure check. - Improve the workforce understanding and acceptance of Autism within Adult Social Care and partner agencies within the Local Authority - Improve the experience of transitions/preparing for adulthood for Autistic young people - To support Autistic people into the pathways into employment enabling them to navigate the support to maximise economic and social wellbeing - To work in partnership with Cornwall's health providers to improve and tackle health inequalities - To build the right support for Autistic people and the families in their communities, with particular focus for Autistic people who have been or at risk of inpatient Mental Health care - To build and strengthen partnership to better support Autistic people who are in contact with, or at risk of entering into the criminal justice system Working Pattern: This is a p...
