About the Role: This position is responsible for leading case management and the delivery of person-centred home and community care services. In this role, you will facilitate the delivery of person-centred quality services to support our clients to live in their homes for longer and stay connected to their local community. The role will be the central point of contact for clients and be responsible for care planning services to support individual needs based on available funding. This is a Permanent Part Time position based in Nambucca Heads, working 24 hours per week Monday to Friday.
