Job description

Description Under the supervision of the Statewide Harm Reduction Manager performs duties relating to the HRC specific community case management and referral services as needed. Tasks/Duties/Responsibilities: 70% Client Services 15% Reporting/Monitoring 10% Hyacinth representation/advocacy 5% Training and certifications - Provide HRC Case Management in designated county. - Conduct Community outreach, provider education and client recruitment of individuals in need of Harm Reduction Services. - Offer case management services to address obstacles while stabilizing at the HRC. - Conduct assessments to determine clients’ needs of supportive services. - Refer clients to HRC staff and/or community agency staff to assist them with psychosocial supports (e.g., housing, transportation, employment). - Follow-up with all referrals and clients to ensure all needs are being met. - Establish, maintain and update client case records using approved agency forms and procedures.
