About Us 360 Health Management is a regional disability support provider servicing Bendigo, Shepparton, Swan Hill and Seymour, offering a range of services to our NDIS participants from Support Coordination, Behaviour Support, Respite and much more. We are looking for people who are eager to take their next step within the industry or those who are new in the world of Support Coordination with no previous experience but want to help people! Responsibilities: - Assist clients in navigating social care services and resources. - Provide administrative support to the social care team, including scheduling appointments and maintaining records. - Conduct initial assessments to identify client needs and develop appropriate support plans. - Collaborate with external agencies and service providers to ensure comprehensive care for clients. - Maintain accurate documentation and case notes in compliance with organisational policies. - Participate in team meetings and training sessions to enhance service delivery and personal development.
