JOB CATEGORY: COMMUNITY HEALTH / CARE MANAGEMENT: POSITION SUMMARY: Under supervision, this position assists the Integrated Care Team (Primary Care Physician, Nurse/Medical Assistant, Care Managers, etc.) by proactively coordinating care activities for patients to facilitate the achievement of quality and cost-effective health outcomes. The focus of this position is working with detention center residents who are transitioning back into the community. REQUIREMENTS: EDUCATION AND/OR EXPERIENCE: - Must self-identify as having lived experience with addiction and recovery and criminal justice involvement. - Must self-identify as being released from jail and having completed community supervision requirements no less than two years ago. - Must be at least 18 years of age. - High school diploma or GED and minimum one year experience delivering peer support services. - Self-starter with strong verbal and written communication skills. - Comfort with change and quality improvement processes. - Experience with Electronic Health Records strongly preferred. - Experience with Motivational Interviewing is strongly preferred. - Awareness of Trauma-informed practices and approaches strongly prefer...
