This role focuses on delivering patient-centered care to a medically complex and often geriatric population, with an emphasis on improving outcomes, reducing hospitalizations, and enhancing quality of life. As a member of the Transitional Care Physicians team, you will collaborate closely with facility staff, physicians, and interdisciplinary teams to ensure seamless, coordinated care. Key Responsibilities: Perform comprehensive patient assessments, including history and physical exams, in SNF, ALF, and ILF settings Diagnose and manage acute and chronic medical conditions Develop and implement individualized care plans Prescribe medications in accordance with Georgia regulations and clinical guidelines Coordinate care with facility staff, specialists, and famili...
