While focused on community-based palliative care, this role will be cross-trained in hospice care to ensure a seamless transition for patients who may need end-of-life services. Essential Duties and Responsibilities: Provides nursing services to patients enrolled in Hospice of Humboldts Community Based Palliative Care Program, including patient assessments, interventions and evaluations. Nursing services will be provided in a variety of locations such as homes, long-term care facilities or via video-conference technology.
