Description Summary: The Licensed Practical Nurse (LPN) at Envision Unlimited plays a vital role in assessing and addressing the medical, health, and safety needs of clients within assigned community living program(s). Under the supervision of a licensed Registered Nurse, the LPN conducts client assessments, documents medical needs and interventions, monitors medication regimens, and collaborates with internal and external partners to ensure client well-being. Essential Responsibilities: - Perform onsite client assessments, intervention and providing treatment - Administers medications including oral, topical and by injection - Perform onsite quality assessment of medication carts - Document client medical needs and interventions, including progress notes. - Monitor medication regimens; review medication records, physician orders, bubble packs, and MARs. - Review client medical records monthly or more frequently as needed. - Communicate effectively with Program Directors, CLS Administrators, the Division Director, physicians, and other medical personnel. - Support client hospitalizations, including admission, discharge planning, and hospital visits as needed. - Travel to multiple C...
