In an integrated care environment, we aim to provide the comprehensive and preventative care each patient needs to be healthier in the new value-based healthcare world. This teammate will be an innovator and experimenter in developing this model and collaborating with several teams across the Village on this executive driven transformation. This position blends innovative strategic thinking with operational execution. Core activities include: creating analyses to generate insights and opportunity areas within our care model, developing clinical and operational process flows to best care for our patients, and creating new field facing tools and reports.
