Patient Flow Navigator - Community Integration

Job Description Reporting to the Manager, Community Integration, the Patient Flow Navigator will collaborate with the members of Interprofessional teams for various initiatives and programs including SCOPE (Seamless Care Optimizing the Patient Experience), Integrated Clinical Pathways, Primary Care Access & Attachment and support priorities for the Primary Care Network (PCN). Our programs and initiatives aim to help reduce avoidable ED visits and hospital re-admissions for patients in need. The Patient Flow Navigator will ensure that the appropriate support is provided to patients, families, and various service providers through education, assessment, intervention and coordination within the care pathway.
