Job description

The Care Coordination Assistant supports the Case Management team in facilitating safe and efficient patient transitions throughout the continuum of care. This role assists with discharge planning, utilization management, authorization support, post-acute care coordination, and communication between healthcare providers, patients, families, payers, and external facilities. The Care Coordination Assistant serves as a key operational support partner to RN Case Managers and Social Workers by coordinating routine post-acute services, managing documentation, arranging transportation, securing authorizations, and ensuring continuity of care for patients transitioning to the next level of care.
