Title: Service Coordinator (Mental Health Case Manager) Job Duties: The Service Coordinator (Mental Health Case Manager) assesses clients to identify individual problem areas necessitating treatment and complete assessments. The Service Coordinator (Mental Health Case Manager) also coordinates the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team.
