Department: COMCARE Pay: $24.88 per hour: Work Schedule: Monday-Friday 8:00am-4:30pm, 40 hours per week. Provide goal-directed, medically necessary services and support to adults who have mental illness and substance abuse problems contributing to repeat hospital admissions, criminal justice system and/or crisis contact. Provide intensive case management to engage consumers, assist with accessing community resources to address basic needs and to identify long term solutions in an effort to promote recovery.
