Department: Status: Benefits Eligible: Yes Hou rs Per Week: 40 Schedule Details/Additional Information: Pay Range $29.00 - $31.00 Major Responsibilities: - Develops competency in clinical practice including appropriate drug therapy for patients and prevention/resolution of drug related problems. Develops competency to evaluate patients for drug-drug, drug-food, drug-lab, and drug-disease interactions. Individualizes drug therapy regimens based on patient’s size, organ functions, and disease states. - Facilitates medication-related continuity of care at admission, transfer and discharge. - Reviews and accurately interprets medication orders. - Develops competency in dispensing and compounding medications consistent with good pharmaceutical practice including compounding technique, packaging, and labeling. Practices in compliance with department policies & procedures, as well as local, state and federal regulations. - Ensures formulary adherence, recommends cost effective therapies to providers, and minimizes waste in daily practice - Actively supports procedures and initiatives to prevent medication events.
