Our Wellness Coaches are responsible for member outreach for those in need of increased support and services as the result of foster care transition, high-risk pregnancy, or mental health conditions. This position will facilitate care coordination with medical and mental health providers and community resources as well as engage with members to ensure goals are achieved, with an emphasis on care for youth engaged in the foster system and pregnant mothers diagnosed with high-risk pregnancies and their newborns.
