What You’ll Do: As a Social Services Specialist, you’ll play a key role in ensuring safe, thoughtful, and well-coordinated transitions of care by: - Meeting with patients and families at admission to complete paperwork and identify psychosocial needs - Coordinating care transitions from admission through discharge in collaboration with the interdisciplinary team - Completing referrals, assessments, care plans, progress notes, and required discharge documentation - Communicating care recommendations and transition plans clearly with patients and families - Providing counseling, education, and advocacy to protect patient rights and support decision-making - Assisting with insurance-related m...
