Job Summary St. Thomas Community Health Center is looking to hire a non medical case manager. The Non-Medical Case Management (NMCM) provides advice and assistance to empower clients to access a variety of necessary services to support medically related outcome measurements. Clients will have varying levels of needs throughout their enrollment in services. These services may include: medical, social, community, legal and financial. Clients who demonstrate a low level of need may benefit from NMCM. Enrollment in NMCM is not always permanent; as a client may move from one type of case management to the other depending on current situations.
