Become a part of our caring community The Field Care Manager, Behavioral Health 2 assesses and evaluates member's needs. We do this to achieve and maintain optimal wellness state. The care manager guides members/families toward resources appropriate for the care and wellbeing of members, facilitating interaction with these resources. The Field Care Manager, Behavioral Health 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action. You will report to Manager, Care Management Behavioral Health. The Field Care Manager, Behavioral Health 2 employs a variety of strategies and techniques to manage a member's physical, environmental and psycho-social health issues. Resolve barriers that hinder care. Ensure patient is progressing towards desired outcomes by managing patient care through assessments and evaluations. May create member care plans. Understand department, segment, and organizational strategy and operating goals, including their linkages to related areas. Please note that field work is a mandatory requirement for this position.
