This clinical position is for an Advanced Heart Failure and Cardiac Transplant Clinical Pharmacist. The Advanced Heart Failure and Cardiac Transplant Clinical Pharmacist will rotate through advanced heart failure and heart transplant service lines, serving as an expert of heart failure and transplant pharmacotherapy for the interdisciplinary HF/Transplant team.
