Overview NaphCare is hiring a Mental Health Discharge Planner to join our team of medical professionals in the Wayne County Jail in Detroit, Michigan . Pay rates range from $22.80 - $24.00 per hour plus shift differentials where applicable! Pay rates range per hour based on employment status, site needs, licensure, and experience. As the Correctional Mental Health Discharge Planner, you will work alongside correctional officers and other medical professionals to ensure high-quality care is provided to the individuals in residence. This is a unique position where you can make a difference to this diverse population of patients with complex mental health and medical needs. Responsibilities: The Mental Health Discharge Planner is responsible for coordinating the transition of patients from the mental health unit back into the community.
