Community Health Navigator Generalist - Pacific Tower (1.0 FTE)

Job description

Purpose The Community Health Navigator Generalist is responsible for providing excellent customer service and care navigation to support Neighborcare Health patients in meeting their care needs. They will conduct outreach to engage Neighborcare patients and help close patient care gaps with targeted outreach, education, and scheduling appointments via call or text. The Community Health Navigator Generalist will also provide basic "one-time touch" care coordination for health-related social needs to improve access to community services and support patient participation in primary care. The Community Health Navigator works in partnership and joint accountability with other Community Health Navigation team members to achieve Neighborcare’s Mission, Guiding Principles and Goals.
