Care Partner - Support at Home

Job description

The Care Partner – Support at Home works closely with older people to coordinate their government funded aged care services, helping them live safely and independently in their own homes. Using a person-centred approach, the role supports consumers to make informed decisions, access appropriate services, equipment and home modifications, and adapt their care as needs change. The Care Partner also coordinates service providers and manages allocated funding to ensure supports align with each individual’s goals, preferences and wellbeing, with guidance from qualified health professionals. This is a permanent, permanent, opportunity working 3 days per week (flexibility to choose days).
