This team is part of a strategy to combat homelessness. The goal of street outreach is to identify, engage, and connect, or re-connect homeless individuals to interim and/or permanent housing and supportive services. The team works closely with each client to develop individualized service plans. The team helps enroll clients in public benefits (i.e. General Relief, SSI/SSDI, Medi-Cal), submit housing applications, and access appropriate medical and behavioral health treatment.
