Job description

Job Purpose or Objective(s): As the Case Manager, you will be a source of social, educational, and spiritual support for individuals and families to develop skills and enhance quality of life. You will report to the Departmental Manager or Director. Primary Tasks: Perform other responsibilities as assigned. - You will contact clients to perform intake assessment and analysis. - Provide an overview to the client by describing the extent and nature of the services available to them. - Provide educational and social case management services to clients including setting goals and developing case plans - Review and monitor participant progress and provide resources, advocacy, and referrals. - You will manage case to ensure progress and adjust as needs change. - Develop working relationships with individual patient and family. - Make referrals to community agencies and conduct follow-up activities. - Be a patient liaison and advocate.
