PROGRAM DESCRIPTION AND JOB SUMMARY: The H2O PTTS Resource Specialist plays a vital role within Primavera’s H2O Housing Pre-Tenancy/Tenancy Sustaining Services program, , providing supportive services for housing searches, lease agreements, system navigation and tenant education to support long-term stability. The role is responsible for ongoing needs assessments, personal treatment plans, implementation, support, information, referrals, and advocacy for individuals who are experiencing homelessness or at risk of homelessness. H2O PTTS Resource Specialist provides housing transition/navigation services and housing pre-tenancy and tenancy-sustaining services. This position requires adaptability to varying routes and schedules, as well as partnering with staff and community partners to provide supportive services to referred participants to increase their self-reliance.
