A Coastal Hospice RN Case Manager is: Responsible for developing, implementing and evaluating a plan of patient care utilizing ANA (American Nurses Association) Standards and the Nursing Process Utilizes equipment and technology skillfully and is technically competent in nursing procedures Oversees and/or participates in safe medication handling and practices Educates patients and families on significant self-care and end of life issues, all hospice services, patient liability for payment and ensures consent and payer forms are completed Acts as a patient advocate. Provides support, information and direct care Triages phone calls to identify patient/family needs and notifies Admission Services of referrals Oversees and delegates assignments of CMTs, LPNs, Hospice Aides, students and volunteers on shift Participates in weekend, holiday, on call...
