The Care Coordinator is responsible for the overall coordination of intensive, family-centered, community-based services. This position facilitates Child and Family Team (CFT) Meetings and guides the evolution of the Plan of Care in order to promote a successful home placement for the referred youth. The Care Coordinator will assist with linkages, resource identification, and other supportive services to effectively promote a successful home placement. The position focuses on accessing and building upon the family’s natural strengths and community resources. This position may also be asked to participate in an after-hours on-call rotation. PROGRAM DESCRIPTION: The Orange County Wraparound program serves children, youth, and non-minor dependents ages 0-21 who are involved with Orange County Social Services, Orange County Healthcare Agency and/or Orange County Probation.
