Job description

The Medication Management Pharmacy Intern is to provide support to team pharmacists, patients, and clinicians to ensure effective, safe, and cost-effective drug therapy. - Data entry: such as obtaining medication histories directly from patients and scheduling visits with a pharmacist. Documents all patient encounters as appropriate in health system documentation system. - Data analysis: such as payer data and claim - Support the pharmacist to implement the patient’s care plan, such as navigating insurance benefits, assisting with provider communication - Provide medication, lifestyle, and disease state counseling - Coaches patients, families, and/or authorized care givers on how to use existing skills and develop new ones to make lifestyle behavior changes that can positively affect patient’s health.
