Summary of Position The Case manager is responsible for managing the VA Transitional Housing Per Diem program in Benton, Stearns, and Sherburne Counties, providing intensive case management services (including Targeted Case Management when eligible and as available) to homeless Veterans subject to the project’s homeless definition. This is a full-time regular position with a workweek expectation of 40 hours. Wage: $20.79-$25.26 per hour, 40 hours a week 400 US Highway 10 S, St. Cloud, MN 56304-1247 Essential Duties and Responsibilities: This job description is not all inclusive.
