Department: 37349 Advocate Good Shepherd Hospital - Navigator Status: Part time Benefits Eligible: No Hou rs Per Week: 0 Schedule Details/Additional Information: as needed for additional support mainly weekdays from 8-4:30, possible weekend event Pay Range: $41.10 - $61.65 Major Responsibilities: Coordination and implementation of strategies to ensure quality, efficacy, and cost effectiveness for the oncology patients in multiple settings. 6)Assesses patient needs 5)Orientates patient to the hospital and outpatient settings as well as the organization 1)The PONN delivers cancer care across the continuum, from diagnosis through treatment. and implements the transition to survivorship, and end-of-life care 2)Using the nursing process, the PONN provides patient and family insight with appropriate education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum 3)Critically thinks and clinically assesses patients providing support and education.
