**$10,000 Sign on Bonus!** BAYADA Home Health Care is seeking a Physical Therapist (PT) for a full time position with our new Central NJ Senior Living team - a Medicare Certified home health office. This office provides best-in-class care to adult and geriatric clients living in communities that include assisted living, independent living, senior congregate living, 55+ communities, and intermediate care facilities for clients with intellectual disabilities throughout Somerset, Middlesex, and Monmouth Counties . *This is a hybrid position that may also include seeing clients in the traditional home health community setting. Responsibilities: Make home visits to clients in designated geographic territories. Qualifications: - Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments. - Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents. - Develop and implement appropriate individualized care plans, including manual thera...
