Licensed Practical Nurse (LPN) We’re looking for an LPN who puts compassion, dignity, and person-centered care at the heart of their work. Schedule: Monday through Friday, with normal business hours, with flexibility to adjust hours earlier or later based on needs of the individuals or scheduled appointments. Location: Faribault, MN: Wage: $27.00 to $30.00 per hour depending on experience. Key Responsibilities: - Provide compassionate, respectful nursing support to individuals in community-based and residential environments. - Deliver nursing care in accordance with LPN licensure, organizational policies, and individual service plans. - Support, coach, and educate direct care staff to promote safe, consistent, and high-quality care. - Ensure medical records and health documentation are accurate, complete, and maintained according to regulatory standards. - Guide staff in recognizing signs and symptoms of illness and in providing appropriate in-home care. - Complete required documentation, including incident reports, program reviews, psychiatric medication monitoring, and daily health logs. - P...
