GRANT FUNDED: OPEN TO THE PUBLIC: SHIFT: Monday-Friday: 8am-5pm WAGE: $28.17: POSTED DATES: 3/9/2026-OUF: Eligible Employer for Public Student Loan Forgiveness as a non-profit Position Summary: - Under the supervision of Public Health Director, responsible for writing the Special Diabetes Program for Indians grant with assistance from grant writing office. Perform all grant reporting. Assist the Public Health Nurse-Diabetes Health Grant with the coordination of services for diabetic clientele. Implement programs and projects to provide better quality services for clients. All Public Health staff must adhere to mandated reporting laws. This is a grant funded position. Essential Duties and Responsibilities: - Provide Public Health Nursing diabetes self-management education services to eligible diabetic patients residing within the five county service area of the Saginaw Chippewa Indian Tribe. - Maintain a diabetes registry of all diabetic, pre-diabetic, and metabolic clients eligible for care and patient recall system for patients within the service area. - Maintain the required diabetes audit tracking system and submit information yearly.
