Description JOB SUMMARY: Within the context of a patient/client and family-centred framework and continuum-based care model and in accordance with established standards of professional practice and the vision, values and policies of the organization, this position plans, develops and implements psychosocial rehabilitation assessment and intervention to clients living in the continuum of supported mental health and substance use housing. Travels to various mental health group home/s and related facilities to provide occupational therapy services to clients living in supported mental health housing and enables clients to achieve and maintain optimal health outcomes by working independently and as a member of an interdisciplinary team. Performs assessments on client’s occupational performance utilizing an evidence-based approach, plans and implements outcome oriented services and evaluates the effectiveness of the services. Provides direction to Mental Health home care staff related to designated client rehabilitation plans and program planning.
