Advanced Practice Providers are key members of the Lifespark Medical Group primary care practice. We deliver primary care to frail and complex seniors living in a variety of settings including Community, independent living, assisted living, memory care, long term care, and transitional care units. We utilize a combination of in-person and telehealth visits inside a multidisciplinary team-based model. Clients and their caregivers receive expert, accessible, and compassionate care. This collaborative and dynamic medical practice is led by industry experts in geriatrics and senior care services. It is delivered under the umbrella of a larger Lifespark enterprise which provides whole person senior care to broad populations of seniors. Lifespark has value-based arrangements with payers which allows clinicians to practice independent of traditional healthcare and payment constraints. These payment methodologies allow clinicians to practice the way they always envisioned when they chose to join the caring profession.
