Job description
Overview The Deputy Director of Care Transformation sits within the Agency of Human Services (AHS) Office of Health Care Reform (HCR) and is responsible for supporting activities related to care transformation, including identifying, developing, and implementing policy and programmatic changes in response to Act 167 and Act 68 to ensure financial sustainability and adequate access across the health system in Vermont. A portion of the role will include overseeing Rural Health Transformation Program (RHTP) activities related to Care Transformation and related day-to-day operations, grants and contracts management, and coordinating with hospitals, providers, and state and federal partners. This position reports to the Director of Care Transformation. CORE RESPONSIBILITIES: Program Leadership: - Support the design and refinement of Vermont's Care Transformation Plan, integrating payment reform, delivery system redesign, and population health goals across statewide, regional, and hospital levels. - Support development and scaling of innovative care delivery models, including alternative service models for rural and underserved areas such as telehealth, mobile care, and regional partners...
