Schedule Full Time, Day Shift (80 hours biweekly) Hours: Monday - Friday 8:00am - 4:30pm General Summary: Provides quality clinical mental health assessments and consultation to providers and supporting clinical care team staff as necessary. They are responsible for triage and management of patients with clinical issues related to their behavioral or psychosocial wellbeing. Additionally, the behaviorist conducts psychosocial assessments, interventions, Transitions of Care (TOC), education, and referrals to inform a holistic treatment plan for patients, in support of Population Health. They are responsible for the management and oversight of a caseload of moderate - high risk Members with complex medical/behavioral /psychosocial needs.
