Job description

Position Summary Under general administrative direction, provides a significant level of patient education related to illness and planned management and treatment and facilitates coordination of care for patients of several divisions within the Medicine department, including Pulmonology, Infectious Diseases, and Renal Medicine. In addition, the Nurse Navigator provides in-clinic support to providers, as needed. Essential Duties and Responsibilities: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Other duties may be assigned. - Liaison for ID patients requiring coordination of care and support. - Work collaboratively with the administrative and clinical ID teams to manage clinical issues; urgent tasks, prescription pre-authorizations and refills, appointments. - Coordinate and manage all urgent orders for dynamic testing and procedure referrals to include patient preparation and education. - Follow up with patients on testing and procedure results in coordination with providers. - Provide appropriate clinical support, counseling, and education.
