Description Sangre de Cristo Community Care is dedicated to compassionate care and innovative programming for patients and families across southern Colorado. We are seeking a visionary leader to champion two cornerstone programs ,our bereavement and volunteer services ,while overseeing our Dementia Care Program, part of CMS’s GUIDE Model to support people living with dementia and their caregivers. The Opportunity: We’re looking for a rare leader, equal parts compassionate clinician, skilled program manager and innovative builder. This role offers the chance to honor tradition while creating something entirely new, helping us expand our reach and impact for some of our most vulnerable communities.
