Job Summary The PRN Community Engagement Specialist is responsible for representing Transitions LifeCare (TL) in the community, strengthening relationships with key community partners, and supporting outreach, education, awareness, and engagement activities across our 11‑county service area. This role blends community presence, event support, outreach relationship-building, and communication support to expand TL’s reach and ensure meaningful connections across diverse communities. The schedule for this position is 10-15 hours per pay period- evenings and weekends. Responsibilities what you do: - Represent TL at community events, health fairs, faith‑based gatherings, partner meetings, and other local engagements. - Serve as a consistent and culturally competent presence in underserved, rural, and diverse communities. - Build and maintain relationships with community organizations, civic groups, senior care providers, and local leaders. - Assist in planning and delivering outreach activities, educational sessions, and special events as needed. - Coordinate logistics, materials, and communication for events and marketing activities. - Support social media and communications efforts wi...
