Job DetailsJob Location: Fargo Headquarters - Fargo, ND 58104Position Type: Per DiemJob Shift: AnyPosition Summary Provision of spiritual care to hospice patients/families in coordination with the interdisciplinary team (IDG) that is consistent with the goals, definitions, philosophy, and purpose of HIA Hospice. This position is PRN position and will provide services in the Fargo area and surrounding areas. Key Responsibilities: Assess the patient’s/family’s need for spiritual care or spiritual care-related services at time of admission and on an ongoing basis. Develops a written plan of care in cooperation with the patient/family and the hospice IDG that identifies needs, outcomes, and interventions for each patient/family Provides chaplaincy visits/services to patient/family as determined in the plan of care. Accurately documents all patient/family contacts and interventions in a timely manner. Provides accurate, efficient patient status reports to the team in IDGs, and in periodic updates.
