Supports Advanced Heart Failure clinic providers and patients. Manages patients with heart failure, pulmonary arterial hypertension, and other complex cardiac disease states. This pharmacist functions as an integrated member of the interdisciplinary care team and works closely with providers, nurses, social workers, and pharmacy technicians to optimize medication therapy, improve access, and enhance patient outcomes. This role is best suited for pharmacists with prior clinical experience or residency training rather than new graduates Pay Range: $54.90 - $82.35 Major Responsibilities: Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens Uses effective patient education techniques to provide counseling to patients and caregivers, including information on medication therapy, adverse effects, compliance, appropriate use, handling, and medication administration Designs, recommends, monitors, and evaluates patient speci...
