Works directly with children and families to provide support and encouragement in their respective environments through case management to address specific needs. Initiates and develops relationship-building opportunities designed to prevent out of home placements and prompts reunification and permanency. DUTIES: SUMMARY: Works directly with children and families to provide support and encouragement in their respective environments through case management to address specific needs. Initiates and develops relationship-building opportunities designed to prevent out of home placements and prompts reunification and permanency.
