Description The Care Coordinator is part of HOPE (Housing Outreach Preventing Eviction), a team that supports clients in Vancouver to maintain their housing. Working within the context of an integrated team-based care model, the Care Coordinator is responsible for coordinating the care of clients that are attached to the HOPE Program and require household management support as a key component of their care plan. HOPE focuses on offering a short-term intervention based on intensive deep-cleaning and/or household management supports that address immediate risks of eviction, life safety issues, or barriers to discharge from hospital. JOB SUMMARY: Within the context of an innovative client centered care model, and in accordance with the standards of practice and the established vision and values of the organization, the Care Coordinator works as part of a multidisciplinary community team using a strengths-based model, comprehensive care plans and discharge planning services to support clients with mental health and substance use concerns.
