Description and Responsibilities Open Sky’s Community Compass Program is a Mass Health funded navigator service that provides case management and care coordination to individuals in the community. Individuals served in the program must have a mental health disorder or co-occurring disorder and be considered at high risk for admission to a 24-hour treatment setting. The Community Compass Navigator will provide case management and direct outreach support to their assigned caseloads, which consist of members enrolled in Community Services Programs, Recovery Support Navigation, Behavioral Health Justice Involved and/or Community Support Program for People Experiencing Chronic Homelessness. Other Key Responsibilities: - Develop a Support Plan with the member that guides services in a way that is consistent and supportive of other existing comprehensive care plans. Complete all Assessments and Support Plans according to prescribed timelines and specifications. - Act as a liaison between members, community agencies, and families to coordinate treatment, including employment and education entities, Care Coordination entities, Emergency Service Providers, outpatient clinics, and others.
