Care Guide Partners PACE is a part of Hosparus Health. As a Program of All-Inclusive Care for the Elderly (PACE) provider, we work to empower people 55 and older with the support they need to stay in the homes they know and love as they age. PACE helps seniors who need nursing home-level care maintain their independence by providing all the medical care and support services they need to continue living comfortably and safely at home. Schedule: PRN (as needed): PACE Transportation Coordinator/Driver Position Summary: Under the supervision of the PACE Transportation Coordinator, the PACE Driver is responsible for the direct transportation of PACE participants to and from their destinations including, but not limited to, transportation to/from the center/day clinic and to/from appointments related to approved PACE covered services. The driver has continuous interaction with participants, caregivers/families and community agencies and plays an integral part in communicating changes in participants to the PACE Transportation Coordinator for input to the Interdisciplinary Team (IDT).
