Key Responsibilities: - Conduct comprehensive primary and preventive care for patients of all ages—including assessment, diagnosis, treatment, and chronic disease management . - Order, interpret, and follow up on diagnostic tests (labs, imaging, etc.). - Prescribe medications and develop individualized care plans. - Provide patient and family education on disease prevention, self-care, and wellness. - Maintain accurate EHR documentation in compliance with state/federal guidelines. - Collaborate with physicians, therapists, and support staff to ensure coordinated care. - Participate in quality improvement, community health initiatives. - Adhere to all clinic policie...
