VNA Care is seeking Home Health RNs for full time Case Manager roles. Schedule: Monday - Friday 8am-4:30pm. No weekends! Territory: Framingham, Natick and surrounding towns. Qualifications: - Must hold a valid MA RN License in good standing - Minimum of 1 year of med/surg experience; acute care experience is beneficial - Ability to work with compassion, organization, and excellent multitasking skills - Willingness to travel within a designated territory Responsibilities: - Conduct home visits to assess patient needs and provide necessary care - Utilize Case Management, Oasis E documentation, Time Management, and EMR system, Home Care Home Base - Collaborate with patients, families, and interdisciplinary team members to develop and implement care plans - Ensure accurate documentation of patient progress and treatment plans Additional Details: To find out more about VNA Care and our commitment to quality patient care, visit vnacare.org.
