Job description

Shift: Days Job type: Per Diem: Hours: 0800-1630: Position Summary: The individual in this position works under the direction of the RN and/or Social Worker to complete referrals for post-acute services for patients and assist with tasks that do not require a clinical license or degree. Physical demands: - Frequent sitting. - Frequent use of fine motor skill (typing/data entry). And reach with arms and hands. - Frequent standing, walking. - Occasional stoop, kneel or crawl. - Lift and/or move up to 20 pounds and occasionally lift and/or move up to 50 pounds. Working Conditions: - May be exposed to infectious diseases. - Interacts with patients and visitors.
